Important Things to Know for 2012-2013

  • PN-GISD ISD Cafeteria plan year is September 1, 2012 through August 31, 2013. Supplemental benefit changes will also become effective September 1, 2012.

  • Online enrollment for the 2012-2013 plans will begin May 1, 2012 and end on May 25, 2012. You must complete your online enrollment within this enrollment period.

  • NBS – National Benefit Services, will be the NEW medical reimbursement account company beginning 9/1/2012. The MGM Benny Card that you currently have will expire on 8/31/2012. NBS will pay claims incurred last plan year, but not submitted until after 9/1/2012. Do not file claims with MGM after 8/31/2012. The new “NBS Flex” card will be sent to your home address 2 to 3 weeks after the first of September 2012, if you have updated your address in the system. The fee for the flex card has been paid for you this year by FBS. Once you receive the NBS Flex Card, do not throw it away. The card is good for several years and can be reloaded with your new election every year. Please plan your contributions carefully, as these accounts are Use it or Lose it! Enrollment is required every year for the Flex Plan, as this benefit does not “rollover”. For details, see the plan brochure by clicking on Benefits & Forms / Reimbursement.

  • CIGNA is the NEW dental carrier effective 9/1/2012. The Cigna dental plan offers a High and Low Option for dental coverage, same plan design as the prior carrier. PN-G ISD pays in full for employee only cost on the Low Option plan and employees can buy-up to the High Option plan if they choose. Employees may choose to add dependents to the dental plan of their choice at their own cost. Late enrollees (employees or their dependents) have a 24 month waiting period for major and Orthodontic services, and benefits will be paid at 50% of the allowable benefit amount.   • Superior Vision is the PN-GISD vision plan provider for 9/1/2012. The plan will not have an increase this year. For details, see the plan information by clicking on Benefits & Forms / Vision.

  • UNUM Disability is offered to employees of PN-GISD. During the annual enrollment period, employees can increase or add disability coverage without providing evidence of insurability! New coverage amounts or changes are subject to pre-existing condition limitations. For details, see the plan brochure by clicking on Benefits & Forms / Disability.

  • ID Watchdog offers Identity Theft Protection to employees and their families. For plan and rate information, click on Benefits & Forms / Identity Theft.

  • CIGNA is the NEW Basic Life & AD&D carrier, and Voluntary Life and Dependent Life Carrier for PN-GISD effective 9-1-2012. This year you can apply up to the guarantee issue amount of $200,000. with NO FORMS to complete or QUESTIONS to ANSWER. Next year (2013) employees enrolling in the plan as late entrants (adding plan or increasing benefit amounts) must complete evidence of insurability to be considered for coverage under this plan (a physical may be required).

  • Plan offerings to PN-GISD ISD employees include: CIGNA Dental Plan, Superior Vision, Unum Educator Select Disability, Philadelphia American Cancer, CIGNA Voluntary Term Life, 5 Star Individual Term Life, ID Watchdog Identity Theft and NBS Flexible Spending Accounts.
