Important Things to Know for 2011-2012
PN-GISD ISD Cafeteria plan year is September 1, 2011 through August 31, 2012. Supplemental benefit changes become effective October 1, 2011.
Online enrollment for the 2012 plan year will begin May 9, 2011 and end on May 20, 2011. You must complete your online enrollment within this enrollment period.
Flexible Spending Accounts (FSA): The plan year for FSA Health and Dependent Care Reimbursement accounts is September 1, 2011 through August 31, 2012. Please plan your contributions carefully, as these accounts are Use it or Lose it!
NEW! Flex Cards: MGM is changing to the “Benny Card” effective 9/1/2011. ALL participants of the medical reimbursement plan will receive two (2) Benny Cards this year. The MGM Flex Card that you currently have will expire on 8/31/2011. Your new Benny Card will be sent to your home address 2 to 3 weeks after the first of September 2011, IF you have updated your address in the system. There is no charge for your initial two cards, but there is a $10.00 fee for each additional dependent card and or replacement of lost/stolen cards. NEW! Dental: MetLife Dental Plan is now offering a High and Low Option for dental coverage. PN-GISD pays in full for employee only cost on the Low Option plan and employees can buy-up to the High Option plan if they choose. Employees may choose to add dependents to the dental plan of their choice at their own cost. Late enrollees (employees or their dependents) have a 6 month waiting period for basic services and a 12 month waiting period for major and Ortho services. Vision: Superior Vision is the PN-GISD vision plan provider effective 10/1/2011. For details, see the plan information by clicking on Benefits & Forms / Vision.
Disability: Pre-existing condition exclusions will apply for new coverage or increases in coverage. For details, see the plan brochure by clicking on Benefits & Forms / Disability.
Identity Theft Protection: is being offered this year through ID Watchdog to employees and their families. For plan and rate information, click on Benefits & Forms / Identity Theft.
Voluntary Term life: Employees enrolling in the plan as late entrants (adding plan or increasing benefit amounts) must complete evidence of insurability to be considered for coverage under this plan (a physical may be required).
